**Liability Waiver** I, the Member/Guardian, agree to the following for participating in Brazilian Jiu-Jitsu, yoga, and breath-work at Gentle Arts Academy: **Assumption of Risk:** I understand these activities involve inherent risks, including injury or illness, and I choose to participate voluntarily. **Release of Claims:** I release Gentle Arts Academy and its affiliates from all claims related to injuries or damages during participation. **Waiver of Liability:** I waive the right to pursue legal action for negligence. **Medical Fitness:** I confirm I am fit to participate and will consult a doctor if needed. **Medical Treatment Consent:** I authorize necessary medical treatment at my expense. **Rules Agreement:** I will follow all safety rules. I consent to the use of my images for promotional purposes. I confirm I am over 18 and not under the influence of substances. By ticking the acknowledgment box, I agree to these terms.
